Male Circumcision

ケニアにおいて、
保健衛生省の進める
HIV/AIDS 戦略

・VCT　(自発的HIV test)
・PITC (医療従事者の勧めるtest)
・PMTCT(母子感染予防)
・ART (抗ウイルス薬による治療)

と並んで、昨年から新たな strategy として組み込まれたのが

・VMMC (男性割礼)
　Voluntary Medical Male Circumcision

日本では馴染みのない習慣ではありますが、
アフリカでは以前から部族の儀式としての割礼があったようです。


MCの利点として、

・Reduce risk of becoming infected with HIV
・Reduce risk of some STIs
・Easier to keep the penis clean
・Reduce risk of urinary treat infections in childfood
・Prevent inflammation of the glans and the foreskin
・Prevent the potential for development of scar tissue on the forskin
・reduce risk of penile cancer
・reduce risk of cancer of the cervix in female sex partners

などがあげられています。
もちろんリスクについての言及もされていて、

・pain
・bleeding
・haematoma
・infection at the site of the circomcision
・increased sensitivity of the glans penis for the
first few months after the procedure
・irritation of the glans
・meatitis
・injury to the penis
・adverse reaction to the anaesthetic used during the circumcision
・scar tormation, including keloids

この VMMC に関する training が来週から２週間の日程で行われます。
counselling の仕方や、benefits, Risks の説明、手術の方法など、
資料を見ているだけでも、内容の濃密さが伺われます。
ケニアの MC の達成率は８０％以上というデータもあり、
多くの人にとって身近なテーマであること言えます。
